Introducing Live Longer Better.

ONLINE WELLBEING PROGRAMME.

A first of its kind digital self-management programme that empowers participants to take charge of their health and wellbeing for the long term.  NHS Devon has commissioned Live Longer Better as part of its Healthy Ageing service in Torbay and South Devon.  This coached version of the programme is designed to help those with multimorbidities.  We need your help to reach those who could benefit most from this ageing revolution.

What’s the overall aim of the programme? 
  1. Increase physical, cognitive and emotional activity by focussing on the three Rs: regain confidence and purpose, recover the strength, stamina, skill and suppleness participants may have lost and recondition the body, brain and mind
  2. Increase healthy life expectancy and compress the period of dependency by delaying slowing or preventing problems such as disability and Dementia which are often blamed on the ageing process
  3. Reduce the need for health and social care because a person with greater strength, stamina, skill and suppleness has greater resilience and is less likely to need acute care and social care

What’s involved? 
  • ​10 digital lessons delivered in bite-size videos
  • Group online learning classroom with opportunity to share and support 
  • 10 ‘actions’ for participants to engage with; designed to build confidence and facilitate positive change 
  • Personalised support from a health coach throughout 

What are the outcomes?
​Participants report an average 19% improvement in both health confidence and quality of life, alongside a 51% increase in moderate exercise participation. This signifies an increase in knowledge, confidence and skills to self-manage health.

Who is behind the Live Longer Better programme in Torbay and South Devon?
​The Live Longer Better programme is led by Muir Gray, and delivered by e-learning specialists Learning with Experts. It is delivered as part of the Healthy Ageing consortium across Torbay and South Devon, led by Torbay Community Development Trust and Teignbridge CVS respectively.
How to refer
​Eligibility criteria: 
  • Two or more long term health conditions 
  • Aged 60+
  • At risk of unplanned hospital admission
  • Access to a computer / tablet with internet for video streaming.  This could be their own device, shared (e.g. in a library) or by sitting alongside a friend, family member or supporter.

​Live Longer Better is there to support those who are becoming aware of their role in their healthcare but are mostly lacking confidence in doing so. Please share the programme aims, benefits and commitments with your patient before sharing the get started link / text message with them. All participants take this first step themselves which is a great way to initiate engagement in the programme.
